How to Submit an Incident Report

Please provide as much detail as possible when submitting your report. Include the date, time, location, and individuals involved in the incident. This information helps our organization review and address concerns appropriately.
All reports are reviewed by the North Henry Athletics Advisory Board. Submissions will be handled professionally and with discretion.
